Activity Permission and Waiver
I provide permission for my child/ student (“Minor”) to participate in the Activity which includes doing various scientific experiments and I confirm that Minor is physically and mentally fit to participate in the Activity safely.
I understand that Minor’s participation in the Activity may include various risks, such as the risk of property loss, accident, illness, and personal injury. In consideration for Minor’s participation in the Activity, I agree to release New England Biolabs, Inc. and its subsidiaries, affiliates, agents, successors, and assigns (“NEB”) from and waive any claim against NEB arising out of Minor’s participation in the Activity.
Image Release and Waiver
I also provide permission for NEB to use, re-use, publish, modify, edit, copy, display, share, broadcast, and prepare derivative works of (“Use”) Minor’s name, image, likeness, photograph, voice, verbal or written comments, opinions, affiliation and/or locality (“Image”) taken during or arising from the Activity, for any purpose, including promotional purposes, in any media form (whether now known or invented in the future), worldwide, and in perpetuity without further credit, restriction, notice, permission, or compensation and at NEB’s sole discretion.
In consideration for Minor’s participation in the Activity, I understand that I waive any expectation of privacy with respect to Use of Minor’s Image and I agree to release NEB from and waive any claim against NEB related to NEB’s Use of Minor’s Image.
